OHA Kanaaho Grant for Lahaina & Kula

The purpose of the OHA Kanaaho Grant is to provide direct financial assistance to beneficiaries to address the impacts of disasters. The grant funds will support the empowerment of impacted Native Hawaiians to determine their own greatest needs, to meet those needs in the face of disaster, and improve the economic stability of Native Hawaiians.

ELIGIBILITY

Disaster Aid funding will be distributed to qualified head of households who meet all of the eligible criteria below:
  1. Applicant must be Native Hawaiian (Verification Required);
  2. Experienced hardship due to disaster;
  3. Provide proof of Residency
  4. Provide proof of Home Ownership or Proof of Rentership
  5. Submit a signed W-9 form

            REQUIREMENTS FOR NON-NATIVE HAWAIIAN PARENT OR GUARDIAN

            (Only for parent or guardian applying on behalf of a Native Hawaiian minor or dependent)
            Dependent's Name
            MUST HAVE: 1) Birth Certificate of dependent; and legal guardianship/adoption documentation for dependent, or DHS 1508 form, paperwork from DHS acknowledging the foster child relationship or power of attorney. Native Hawaiian dependent must meet the Native Hawaiian verification requirements. 2) Please use the parent or guardian’s information for the application. Parent or guardian must meet all other requirements: proof of residency, proof of ownership or rental status, and signed W-9.
            Upload legal guardianship/adoption documentation for dependent, power of attorney, DHS 1508 form, paperwork from DHS acknowledging the foster child relationship. Native Hawaiian dependent must meet the Native Hawaiian verification requirements.
